Stomach bruising after crash — how long does this actually last??

So I got rear-ended pretty hard about 5 days ago by someone who blew through a stop sign. Not my fault at all — there's a witness and everything — but that's almost secondary right now because I just feel awful physically.

My main issue is my abdomen. The seatbelt locked up hard on impact and I have this deep bruising across my lower stomach and hip area. Went to the ER the same day, they did imaging, and thankfully no internal bleeding or organ damage — just what the doctor called "soft tissue and internal bruising." They sent me home with some pain meds and basically said follow up with your doctor.

But here's the thing — it's been almost a week and I still:

  • Can barely eat without feeling sick
  • Have this constant dull ache that gets sharp if I move wrong
  • Feel like my whole core is just... broken
  • Can't sleep on my stomach or side at all

The outside bruising looks gnarly and keeps changing color (yellowing now which I guess is healing?). But the inside aching is what's really getting to me.

Has anyone else dealt with this kind of bruising from a seatbelt or impact? How long did it take before you felt like a normal person again? I'm supposed to go back to work next week and honestly can't imagine sitting at a desk for 8 hours.

Also — should I be documenting all of this somehow for insurance purposes? I don't really know what I'm doing on that side of things yet.

    Oh man, I felt this post deeply. I had almost the exact same thing happen — seatbelt bruising across my abdomen after a bad intersection crash. The yellowing is definitely a good sign, that means it's moving through the stages. For me the deep internal ache lasted about 3 weeks before it stopped being a constant thing. Around week 2 it shifted from "always there" to "only when I move weird." Hang in there, it does get better, just slower than you want it to.

    The nausea and "off" feeling in your gut after that kind of impact is really common even without organ damage. Your abdominal muscles took a serious hit and everything in there is inflamed and unhappy. The color progression you're describing — deep purple/red to yellow-green — is normal healing. Internal bruising can honestly take 4–6 weeks to fully resolve, sometimes longer depending on how deep it was. The "need to poop" feeling is often just referred discomfort from bruised muscle layers pressing on things.

    That said: if the pain suddenly gets worse instead of gradually better, or you develop a fever, go back in. Don't tough it out if something feels wrong. And yes — document everything. Photos with timestamps, a daily symptom journal, all of it.

    Please please please do not give a recorded statement to the other driver's insurance before you know the full extent of your injuries. They will call you soon if they haven't already, and they will sound super friendly and sympathetic. Don't let them close your claim out while you're still healing. Once you sign anything it's basically over.

    Jumping on what the person above said — I used to work claims and the early-contact calls are very much a strategy. The goal is often to get you to settle before you realize how long recovery actually takes or before any treatment costs pile up. "Soft tissue" injuries are notoriously underpaid in quick settlements. I'm not saying don't cooperate at all, but there's a difference between basic cooperation and signing a release. Know the difference.

    To answer your documentation question — yes, start now if you haven't. A few things worth doing: (1) take timestamped photos of bruising every couple days as it changes, (2) keep a simple notes app log of your symptoms and pain level each day, (3) save every medical bill, co-pay receipt, even parking at the hospital, (4) if you miss work, document that too.

    This isn't about being litigious, it's just that memories fade and insurance companies love to argue about the severity of injuries months down the line. Having a contemporaneous record helps a lot.

    I'm so sorry you're going through this 😔 Please don't push yourself to go back to work before you're ready just because of the schedule. Your body went through something traumatic and it deserves actual recovery time. Is there anyone who can help you out at home this week?

    Two things: (1) Call your doctor before next week's work return and get their actual medical opinion on whether you're cleared — don't just guess. If they say you're not ready, get that in writing. (2) Talk to a PI attorney before you talk to any insurance adjuster further. Most do free consultations and it costs you nothing to understand your options. You don't have to hire anyone, but you should know what you're dealing with first.

    The fact that imaging came back clean is genuinely huge — I know it doesn't feel like good news when you're in pain, but it rules out a lot of scary stuff. Your body is doing the work it's supposed to do right now. The yellowing bruise is literally your immune system doing its job. Slow and frustrating, but it IS progress.
